Treatment of acute venous thromboembolism with dabigatran or warfarin and pooled analysis.

BACKGROUND: Dabigatran and warfarin have been compared for the treatment of acute venous thromboembolism (VTE) in a previous trial. We undertook this study to extend those findings. METHODS AND RESULTS: In a randomized, double-blind, double-dummy trial of 2589 patients with acute VTE treated with low-molecular-weight or unfractionated heparin for 5 to 11 days, we compared dabigatran 150 mg twice daily with warfarin. The primary outcome, recurrent symptomatic, objectively confirmed VTE and related deaths during 6 months of treatment occurred in 30 of the 1279 dabigatran patients (2.3%) compared with 28 of the 1289 warfarin patients (2.2%; hazard ratio, 1.08; 95% confidence interval [CI], 0.64-1.80; absolute risk difference, 0.2%; 95% CI, -1.0 to 1.3; P<0.001 for the prespecified noninferiority margin for both criteria). The safety end point, major bleeding, occurred in 15 patients receiving dabigatran (1.2%) and in 22 receiving warfarin (1.7%; hazard ratio, 0.69; 95% CI, 0.36-1.32). Any bleeding occurred in 200 dabigatran (15.6%) and 285 warfarin (22.1%; hazard ratio, 0.67; 95% CI, 0.56-0.81) patients. Deaths, adverse events, and acute coronary syndromes were similar in both groups. Pooled analysis of this study RE-COVER II and the RE-COVER trial gave hazard ratios for recurrent VTE of 1.09 (95% CI, 0.76-1.57), for major bleeding of 0.73 (95% CI, 0.48-1.11), and for any bleeding of 0.70 (95% CI, 0.61-0.79). CONCLUSION: Dabigatran has similar effects on VTE recurrence and a lower risk of bleeding compared with warfarin for the treatment of acute VTE. BACKGROUND: Whether the oral factor Xa inhibitor edoxaban can be an alternative to warfarin in patients with venous thromboembolism is unclear. METHODS: In a randomized, double-blind, noninferiority study, we randomly assigned patients with acute venous thromboembolism, who had initially received heparin, to receive edoxaban at a dose of 60 mg once daily, or 30 mg once daily (e.g., in the case of patients with creatinine clearance of 30 to 50 ml per minute or a body weight below 60 kg), or to receive warfarin. Patients received the study drug for 3 to 12 months. The primary efficacy outcome was recurrent symptomatic venous thromboembolism. The principal safety outcome was major or clinically relevant nonmajor bleeding. RESULTS: A total of 4921 patients presented with deep-vein thrombosis, and 3319 with a pulmonary embolism. Among patients receiving warfarin, the time in the therapeutic range was 63.5%. Edoxaban was noninferior to warfarin with respect to the primary efficacy outcome, which occurred in 130 patients in the edoxaban group (3.2%) and 146 patients in the warfarin group (3.5%) (hazard ratio, 0.89; 95% confidence interval [CI], 0.70 to 1.13; P<0.001 for noninferiority). The safety outcome occurred in 349 patients (8.5%) in the edoxaban group and 423 patients (10.3%) in the warfarin group (hazard ratio, 0.81; 95% CI, 0.71 to 0.94; P=0.004 for superiority). The rates of other adverse events were similar in the two groups. A total of 938 patients with pulmonary embolism had right ventricular dysfunction, as assessed by measurement of N-terminal pro-brain natriuretic peptide levels; the rate of recurrent venous thromboembolism in this subgroup was 3.3% in the edoxaban group and 6.2% in the warfarin group (hazard ratio, 0.52; 95% CI, 0.28 to 0.98). CONCLUSIONS: Edoxaban administered once daily after initial treatment with heparin was noninferior to high-quality standard therapy and caused significantly less bleeding in a broad spectrum of patients with venous thromboembolism, including those with severe pulmonary embolism. BACKGROUND: Apixaban, an oral factor Xa inhibitor administered in fixed doses, may simplify the treatment of venous thromboembolism. METHODS: In this randomized, double-blind study, we compared apixaban (at a dose of 10 mg twice daily for 7 days, followed by 5 mg twice daily for 6 months) with conventional therapy (subcutaneous enoxaparin, followed by warfarin) in 5395 patients with acute venous thromboembolism. The primary efficacy outcome was recurrent symptomatic venous thromboembolism or death related to venous thromboembolism. The principal safety outcomes were major bleeding alone and major bleeding plus clinically relevant nonmajor bleeding. RESULTS: The primary efficacy outcome occurred in 59 of 2609 patients (2.3%) in the apixaban group, as compared with 71 of 2635 (2.7%) in the conventional-therapy group (relative risk, 0.84; 95% confidence interval [CI], 0.60 to 1.18; difference in risk [apixaban minus conventional therapy], -0.4 percentage points; 95% CI, -1.3 to 0.4). Apixaban was noninferior to conventional therapy (P<0.001) for predefined upper limits of the 95% confidence intervals for both relative risk (<1.80) and difference in risk (<3.5 percentage points). Major bleeding occurred in 0.6% of patients who received apixaban and in 1.8% of those who received conventional therapy (relative risk, 0.31; 95% CI, 0.17 to 0.55; P<0.001 for superiority). The composite outcome of major bleeding and clinically relevant nonmajor bleeding occurred in 4.3% of the patients in the apixaban group, as compared with 9.7% of those in the conventional-therapy group (relative risk, 0.44; 95% CI, 0.36 to 0.55; P<0.001). Rates of other adverse events were similar in the two groups. CONCLUSIONS: A fixed-dose regimen of apixaban alone was noninferior to conventional therapy for the treatment of acute venous thromboembolism and was associated with significantly less bleeding (Funded by Pfizer and Bristol-Myers Squibb; ClinicalTrials.gov number, NCT00643201).

CONTEXTE: Un régime à dose fixe de rivaroxaban, un inhibiteur du facteur Xa par voie orale, a été montré pour être aussi efficace qu'un traitement anticoagulant standard pour le traitement de la thrombose veineuse profonde, sans la nécessité d'une surveillance en laboratoire. Cette approche peut également simplifier le traitement de l'embolie pulmonaire. Méthodes: Dans une, open-label, essai de non infériorité event-driven randomisé portant sur 4832 patients ayant subi une embolie pulmonaire symptomatique aiguës avec ou sans thrombose veineuse profonde, nous avons comparé le rivaroxaban (15 mg deux fois par jour pendant 3 semaines, suivie par 20 mg une fois par jour) avec un traitement standard par énoxaparine suivie par un antagoniste dose ajustée vitamine K pendant 3, 6 ou 12 mois. Le critère principal d'efficacité était la maladie thromboembolique veineuse symptomatique récurrente. Le résultat principal de sécurité était importante ou des saignements de nonmajor cliniquement pertinente. RÉSULTATS: Le rivaroxaban était non inférieur au traitement standard (marge de non-infériorité, 2,0, p = 0,003) pour le critère principal d'efficacité, avec 50 événements dans le groupe rivaroxaban (2,1%) versus 44 événements dans le groupe de thérapie standard (1,8%) (risque ratio 1,12, intervalle de confiance à 95% [IC], 0,75 à 1,68). Le résultat principal de sécurité a eu lieu dans 10,3% des patients dans le groupe rivaroxaban et 11,4% de ceux dans le groupe de thérapie standard (hazard ratio 0,90, IC 95%, 0,76 à 1,07, p = 0,23). Les saignements majeurs a été observée chez 26 patients (1,1%) dans le groupe rivaroxaban et 52 patients (2,2%) dans le groupe de thérapie standard (hazard ratio 0,49, IC 95%, de 0,31 à 0,79, p = 0,003). Tarif des autres événements indésirables étaient similaires dans les deux groupes. CONCLUSIONS: un régime à dose fixe de rivaroxaban seul était non inférieur au traitement standard pour le traitement initial et à long terme de l'embolie pulmonaire et a eu une amélioration potentielle profil bénéfice-risque. CONTEXTE: Le traitement de l'embolie pulmonaire avec de l'héparine de faible poids moléculaire et des antagonistes de la vitamine K comme la warfarine, n'est pas idéal. Nous avons cherché à évaluer la non-infériorité de idrabiotaparinux, un inhibiteur réversible longlasting indirect du facteur X activé, à la warfarine chez les patients ayant une embolie pulmonaire aiguë symptomatique. Méthodes: Dans notre, en double aveugle et double placebo, essai de non-infériorité randomisée, nous avons recruté des adultes avec l'embolie pulmonaire aiguë symptomatique objectivement documenté fréquentant les centres 291 dans 37 pays. Nous avons exclus les patients qui étaient enceintes, eu un saignement actif, une insuffisance rénale ou d'hypertension maligne, ou étaient à risque élevé de décès, saignement, ou de réactions indésirables aux médicaments étudiés. Nous avons réparti au hasard les patients à recevoir l'énoxaparine 1.0 mg / kg de 5-10 jours deux fois par jour, suivie par voie sous-cutanée idrabiotaparinux (dose initiale de 3,0 mg) ou de warfarine à dose ajustée (rapport international normalisé 2.0 -3 · 0) ; schémas duré 3 mois ou 6 mois dépendant de la présentation clinique. Bloquer randomisation a été réalisée avec un système informatisé interactif central, stratifiées par centre d'études et la durée de traitement prévue. Le critère principal d'efficacité était la thromboembolie veineuse récidivante à 99 jours après la randomisation. Nous avons estimé les odds ratios et IC à 95% avec un χ (2) analyse Mantel-Haenzsel (marge de non-infériorité 2.0) dans l'intention de traiter la population. Le résultat principal de sécurité était saignements cliniquement importants (majeur ou non majeur) chez tous les patients au jour 99. Cette étude est inscrit sur ClinicalTrials.gov, numéro NCT00345618. RÉSULTATS: entre le 1 août 2006 et le 31 janv. 2010, nous avons recruté 3202 patients âgés de 18 à 96 ans. 34 (2%) sur 1599 patients répartis au hasard pour recevoir l'énoxaparine idrabiotaparinux et 43 (3%) sur 1603 patients répartis au hasard pour recevoir l'énoxaparine-warfarine a thromboembolie veineuse récidivante (odds ratio 0,79, IC 0,50 -1 95% · 25; p (non-infériorité) = 0,0001). 72 (5%) sur 1599 patients dans le groupe énoxaparine idrabiotaparinux et 106 (7%) sur 1603 patients dans le groupe énoxaparine-warfarine eu des saignements cliniquement pertinente (0.67, 0.49 -0 · 91; p (supériorité) = 0,0098). On remarqué des différences similaires au niveau des résultats dans les patients traités 6 mois. Interprétation: Idrabiotaparinux pourrait offrir une alternative intéressante à la warfarine pour le traitement à long terme de l'embolie pulmonaire, et semble être associé à des saignements réduite. Prospective comparison of three enoxaparin dosing regimens to achieve target anti-factor Xa levels in hospitalized, medically ill patients with extreme obesity.

Enoxaparin is commonly used to prevent venous thromboembolism(VTE) [1,2] but has not been well-studied in patients with extreme obesity,a population at high risk for VTE. We prospectively compared three enoxaparin dosing regimens for the achievement of goal peak anti-Factor Xa levels in medically ill patients (n 5 31) with extreme obesity (body mass index (BMI) ‡ 40 kg/m2). Patients were assigned to receive fixed-dose (FD) enoxaparin 40 mg daily (QDay, n 5 11), weight based,lower-dose (LD) enoxaparin 0.4 mg/kg QDay (n 5 9), or weight based,higher-dose (HD) enoxaparin 0.5 mg/kg QDay (n 5 11). The average BMI and weight of the entire cohort was 62.1 kg/m2 (range40.5–82.4) and 176 kg (range 115–256 kg) and did not differ between groups. Peak anti-Factor Xa levels were significantly higher in the HD group compared to either LD or FD groups. Patients in the HD group achieved target anti-Factor Xa levels more frequently than the LD and FD groups (P < 0.05). Peak anti-Factor Xa levels did not correlate with age, weight, BMI, or creatinine clearance, demonstrating the predictability of weight-based enoxaparin dosing. There were no adverse events (e.g., bleeding, thrombosis, thrombocytopenia). To our knowledge this is the first prospective comparative study demonstrating that in extremely obese, medically ill patients enoxaparin 0.5 mg/kg QDay is superior to FD and LD enoxaparin for the achievement of target anti-Factor Xa levels.

Safety profile of tinzaparin versus subcutaneous unfractionated heparin in elderly patients with impaired renal function treated for acute deep vein thrombosis: the Innohep® in Renal Insufficiency Study (IRIS).

INTRODUCTION: Trials comparing the use of full dose unfractionated heparin (UFH) or low molecular weight heparins (LMWHs) in very elderly patients with impaired renal function are lacking. IRIS aimed to assess whether LMWH is at least as safe as UFH in this population. MATERIALS AND METHODS: The study included renally impaired patients ≥70 years with acute symptomatic lower limb deep vein thrombosis (DVT). Patients were randomized to initial treatment with either tinzaparin 175 IU/kg once daily (n=269) or activated partial thromboplastin time-adjusted UFH twice daily (n=270). After acute management both groups received vitamin K antagonist to day 90. RESULTS: The trial was stopped prematurely due to a difference in mortality favoring the UFH group (11.5 vs. 6.3%; p=0.035). Rates of clinically relevant bleedings by day 90 were similar in the tinzaparin (11.9%) and UFH (11.9%) groups, as were rates of confirmed recurrent venous thromboembolism (VTE) (2.6 vs. 1.1%; p=0.34). As the mortality difference could not be explained by bleedings or recurrent VTE, a post-hoc analysis was performed. This identified six baseline characteristics significantly correlated with mortality, of which five were over-represented in the tinzaparin group. CONCLUSION: The IRIS study was a challenging study involving patients (mean age 83 years) usually excluded from clinical studies, but its early termination has left questions unanswered. The mortality difference observed with tinzaparin vs. UFH in elderly, renally-impaired patients with DVT cannot be explained on the basis of bleedings or recurrent VTE, and may reflect an imbalance of mortality risk factors at baseline.

CONTEXTE: Bien que les guides de pratique recommandent soins ambulatoires pour certains, patients hémodynamiquement stables présentant une embolie pulmonaire, la plupart des traitements en milieu hospitalier est actuellement basé. Nous avons cherché à évaluer la non-infériorité de soins ambulatoires par rapport aux soins hospitaliers. MÉTHODES: Nous avons entrepris une étude ouverte, randomisée de non-infériorité d'essai de 19 services d'urgence en Suisse, en France, en Belgique et aux Etats-Unis. Nous assignés au hasard des patients en phase aiguë, une embolie pulmonaire symptomatique et d'un faible risque de mortalité (embolie pulmonaire classes de sévérité indice de risque I ou II) avec un ordinateur généré séquence de randomisation (blocs de 2-4) dans un rapport de 1:1 à ambulatoire initial (c.-à-congé de l'hôpital ≤ 24 h après la randomisation) ou un traitement avec hospitalisation énoxaparine sous-cutanée (≥ 5 jours), suivie par une anticoagulation orale (≥ 90 jours). Le critère principal était symptomatique, la thromboembolie veineuse récurrente dans les 90 jours; résultats de sécurité inclus saignements majeurs dans les 14 ou 90 jours et la mortalité dans les 90 jours. Nous avons utilisé une marge de non-infériorité de 4% pour une différence entre les patients hospitalisés et les groupes externes. Nous avons inclus tous les patients inscrits dans l'analyse principale, à l'exclusion des perdus de suivi. Ce procès est inscrit sur ClinicalTrials.gov, NCT00425542 nombre. RÉSULTATS: Entre Février 2007 et Juin 2010, nous avons recruté 344 patients admissibles. Dans l'analyse principale, un (0,6%) de 171 patients ambulatoires développé thromboembolie veineuse récurrente dans les 90 jours, comparativement à aucun de 168 patients hospitalisés (95% de limite de confiance supérieure [UCL] 2,7%, p = 0,011). Un seul (0,6%) patients dans chaque groupe de traitement est décédé dans les 90 jours (95% UCL 2,1%, p = 0,005), et deux (1,2%) de 171 patients ambulatoires et hospitalisés eu aucun saignement majeur dans les 14 jours (95% UCL 3,6%, p = 0,031). En 90 jours, trois (1,8%) patients non hospitalisés, mais aucun des patients hospitalisés avaient développé des saignements majeurs (95% UCL 4,5%, p = 0,086). La durée moyenne de séjour était de 0,5 jours (SD 1,0) pour les patients externes et de 3,9 jours (SD 3,1) pour les patients hospitalisés. Interprétation: Dans certaines patientes à faible risque d'embolie pulmonaire, les soins ambulatoires peuvent en toute sécurité et efficacement être utilisés à la place des soins hospitaliers. BACKGROUND: Unfractionated heparin (UFH) is the standard drug for the initial treatment of pulmonary embolism (PE) and deep vein thrombosis (DVT) in Japan, whereas fondaparinux is the standard drug in Europe and the United States. Here, we examine the efficacy and safety of fondaparinux in Japanese patients. METHODS AND RESULTS: In 2 randomized, open-label, multicenter studies, 80 Japanese patients with acute PE or DVT received either subcutaneous fondaparinux or intravenous UFH as a non-comparative reference, in a 3:1 ratio, for 5-10 days. Concomitant warfarin therapy was continued until Day 90. Multidetector-row computed tomography-based assessment showed that 57.9% and 45.9% of the patients with acute PE and acute proximal DVT had proximal DVT and PE as a complication, respectively. There was no recurrence of symptomatic venous thromboembolism. In the fondaparinux group, the respective improvement rates at the end of the initial treatment and follow-up periods were 71.4% and 86.8% for 42 patients with PE, and 57.8% and 83.3% for 46 patients with DVT; similar results were noted in the UFH group. One patient in the fondaparinux group experienced major bleeding during the initial treatment, but no such episode in the UFH group. CONCLUSIONS: Once-daily, subcutaneous fondaparinux is as effective and safe without monitoring as adjusted-dose intravenous UFH for the initial treatment of acute PE and DVT in Japanese patients.
